Risk Warning - Dangerous Recreational Activity WARNING: Under the Civil Liability Act, contrast therapy involving extreme temperature exposure is classified as a dangerous recreational activity.

Participation involves significant risks including but not limited to:

• Thermal shock and extreme temperature stress

• Cardiovascular stress and blood pressure fluctuations

• Dizziness, fainting, or loss of consciousness

• Slips and falls on wet surfaces

• Hypothermia or hyperthermia

• Breathing difficulties

• Dehydration and electrolyte imbalance

• Exacerbation of existing medical conditions

Participant Declaration and Acknowledgment

I acknowledge and agree that:

1. I have been informed of and understand the risks involved in contrast therapy

2. I voluntarily participate at my own risk

3. I am physically fit and have no medical conditions that would prevent safe participation (or have disclosed them below)

4. I will immediately notify staff if I feel unwell, dizzy, or experience any discomfort

5. I will follow all safety instructions and facility rules

6. I understand I can exit the therapy at any time

7. I have had the opportunity to ask questions about the risks

Release and Waiver of Liability

To the extent permitted by law, I release and discharge Thirdspaces, its owners, directors, employees, and agents from all claims, demands, and proceedings arising from:

• Personal injury or death caused by the ordinary risks of contrast therapy

• Loss or damage to personal property

• Any consequential loss

This release does not apply to claims arising from reckless conduct or gross negligence by Thirdspaces.

Indemnity

I agree to indemnify Thirdspaces against any claims brought by third parties arising from my participation in contrast therapy.

Personal Property

I acknowledge that Thirdspaces is not responsible for loss or damage to personal belongings.

Certification and Consent I certify that:

• I have read and understood this entire document

• I have been given adequate time to consider its contents

• My questions have been answered to my satisfaction

• I am over 18 years of age

• I sign this agreement freely and voluntarily
